
Getting Started at Mindful Mending, LLC

Prior to you first appointment you will be asked to connect to our Client Portal, an easy way to access your mental health records. Paperwork that is sent must be completed at least 6 hours before your first appointment or your session will need to be rescheduled. If you do not receive a link to our paperwork please email us at info@mindfulmendingllc.com and we can resend it.

We have clinicians accepting Aetna, Cigna, and Optum. Coverage will depend on your insurance plan. Please contact your insurance provider directly to learn more about whether your sessions will be covered in part or in full by your provider.
For other insurance plans not accepted at Mindful Mending, LLC, we may be able to provide you with a superbill that you may use to file a claim for out-of-network benefits. Please note it is your responsibility to file out-of-network claims, we do not file claims for out-of-network plans or accept payment from insurers who are out of network.

Mindful Mending, LLC requires a 24 hour cancellation for appointments. If an appointment is canceled in less than 24 hours then there will be a $100 fee. Missed appointment fees will be automatically charged to the credit card you have left on file for us and have given permission for us to charge.Please note that if you late cancel or miss 3 or more appointments, Mindful Mending, LLC may decide to terminate treatment. You will be provided with outside referrals for continued care. If no contact is received from you in a 30-day period, it will be assumed that you are no longer interested in services and your case will be closed.

Notice of Rights for Private Pay Patients
You have the right to receive a “Good Faith Estimate” explaining how much your medical care will cost.
Under the law, health care providers need to give patients who do not have insurance or who are not using insurance an estimate of the bill for medical items and services.
You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. This includes related costs like medical tests, prescription drugs, equipment, and hospital fees.
Make sure your health care provider gives you a Good Faith Estimate in writing at least 1 business day before your medical service or item. You can also ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule an item or service.
If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill within 120 days of receipt of your initial bill.
Make sure to save a copy or picture of your Good Faith Estimate.
